    A case of nonconvulsive status epilepticus presenting as dissociative fugue

    Nonconvulsive status epilepticus is characterised by changes in behaviour, memory, affect or level of consciousness. We report a case of nonconvulsive status epilepticus precipitated by carbamazepine that presented as dissociative fugue. The patient was a 49-year-old man. He first experienced a tonic-clonic seizure nine years previously and was diagnosed with grand mal epilepsy. He had been using carbamazepine 800 mg daily for the last eight years. He had not had any tonic-clonic seizures since carbamazepine was introduced but began to display behavioural disturbances periodically. He also left home many times, and remembered nothing about the period he had been away after he returned. He was hospitalised with a preliminary diagnosis of dissociative fugue. He had recurrent episodes with dissociative symptoms during his stay in the hospital. An electroencephalogram (EEG) and a 24-hour video EEG revealed nothing abnormal. An EEG recording taken during an episode indicated complex partial status epilepticus. Carbamazepine was substituted with valproate, and the patient was discharged in remission

    A strong earthquake with a magnitude of 7.5 occurred near the island of Paramushir (KurilIslands) on 25 March 2020. It caused a weak tsunami in Kamchatka and the Kuril Islands.Earthquake and tsunami data from three DART buoys are discussed and compared withnumerical simulations. It is shown that the calculated and measured tsunami characteristics onthe DART buoys is in very good agreement. There are also data on the recording of thisearthquake by a laser strain-meter installed in the Sea of Japan at Shults cape at a distance ofmore than 2,000 km from the epicenter of the earthquake. There is also an instrumentalrecording of the tsunami at the Vodopadnaya point in the southeast of Kamchatka.Unfortunately, there was a large storm at sea at this time, and the amplitudes of tsunami wavesand storm waves were comparable to each other, so here the agreement between calculationsand observations does not seem good enough

    © 2022 Wiley Periodicals LLC.Background: Prostate multiparametric magnetic resonance imaging (mpMRI) is a useful tool for the detection of tumor lesions however, some clinically significant lesions are still missed. We determined whether the cribriform pattern has an effect on lesion detection in mpMRI. Methods: We reviewed the single-institution database of the patients who underwent mpMRI before radical prostatectomy. We included the patients only with the Gleason 7 final pathology of open radical prostatectomy with curative intent between 2016 and 2021. Prostatectomy mappings according to the 16-sector map and cribriform patterns were re-evaluated by two genitourinary pathologists. Prostate mpMRIs were read by two genitourinary radiologists. If the index and nonindex lesions in pathology mapping were matched with mpMRI as Prostate Imaging Reporting and Data System-3 or higher, it was defined as detectable. We compared the detection rates of lesions with and without cribriform morphology. In regression analysis, we also assessed the factors affecting the detectability of prostate cancer lesions. Results: A total of 120 patients and 157 lesions were included in our study. While 52 of 83 cribriform pattern positive lesions could be detected in mpMRI, 59 of 74 cribriform pattern negative lesions could be detected (62.7% vs. 79.7%, respectively, p = 0.019). The lesions were also distributed homogeneously according to diameters and analyzed separately. All lesions between 21 and 30 mm with the negative cribriform pattern were detected on mpMRI. However, only 77.8% of cribriform pattern positive lesions between 21 and 30 mm could be detected (p = 0.034). The Higher D'Amico risk group and the absence of cribriform morphology were independent predictors for the lesion detection on mpMRI. Conclusion: The presence of cribriform pattern in Gleason 7 prostate cancer lesions decreases the lesion detection rate of mpMRI

Materials and Methods: Cell viability and IC50 values were detected by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay, protein expressions were determined by Western blot and caspase activities were analyzed by activity kit. Results: Western blot analysis showed down-regulation of the cyclin D1, c-Myc, and p53 protein activities, and up-regulation of p27(KIP1) activity after flavopiridol treatment. Additionally, flavopiridol diminished p-Akt protein levels generally which induces inhibition of proliferation. Conclusion: The present study demonstrated that flavopiridol did not induce caspase-3/7 activation, BIM, and BAX pro-apoptotic proteins but it leads to the expression changes of various proteins that inhibit proliferation and eternity in glioblastoma cell lines which have different genetic alterations

However it hasn't database systems for patient files. In this study will provide to web based construct 6893 patient records which are tracking by Ege University faculty of medicine, group of study of reflux. There is no determined reflux software anywhere that is planned software. Therefore this software has a potential that may be used national or international units. The data can be transform interpretable data and transport to designed database that has come from patient tracking system. A data warehouse can be construct for web based decision support system that will develop by acquired data. With data mining algorithms will provide a decision support system that can give their analysis results to the physician. And it will provide a mechanism to perform data analysis. All kept in the archives of patient demographic information, lab results and tests in accordance with the patient information with security rules will be collected in a common database. This system will allow personnel of health to easily access the system by using different platforms. It also provides for entry of up-to-date information and rapid queries of patient information while the treatment process. This software will provide academic, operational, technological and economical a lots of benefits

    Prognostic value of FDG PET-CT in suspected recurrence of colorectal carcinoma: survival outcomes of a 10-year follow-up FDG PET in recurrent colorectal CA

    Objective We aimed to evaluate the predictive value of FDG PET-CT scan and CEA measurements in recurrent colorectal cancer (CRC) patients. Methods The records of 211 CRC patients who had FDG PET-CT scans between April 2009 and June 2011 due to suspicion of recurrence were extracted from the data of our previous report of 235 patients after 24 patients were excluded from the study due to lack of follow-up data or death unrelated to CRC. FDG PET-CT findings, simultaneous CEA levels, and survival data were evaluated retrospectively to determine the prognostic factors that affected the overall survival (OS) of the patients. Results The mean age of 211 patients was 60.2 +/- 12.8 years. The median follow-up time was 39 months (CI 95%: 4-123 months). The CRC-related death rate was 71.6% and the median OS time was measured 39 months (CI 95%: 27-50 months) for 211 patients. The median OS time for the patients with positive findings for recurrence in PET scans was 28 months (CI 95%: 22-33 months) which was significantly shorter (p < 0.001) than that of PET-negative patients (median OS was not reached; mean OS: 105 months; CI 95%: 95-116 months). CEA positivity also had a significant negative effect on survival (p < 0.001). Median OS times in patients with elevated and normal levels of CEA were 24 months (CI 95%: 17-30 months) and 85 months (CI 95%: 62-107 months), respectively. When the effect of CEA positivity was evaluated in patients with negative PET scans for recurrence, no statistically significant difference was determined (p = 0.209), but PET positivity had a significant negative effect on OS in patients with normal levels of CEA (p < 0.001). On the other hand, PET negativity had a significant positive effect on OS in patients with elevated CEA levels (p = 0.002). The extend of recurrent disease had also a significant effect on OS. The patients with distant metastasis had less favorable OS than those patients with only local recurrence (p < 0.001). The presence of liver metastasis also diminished the OS, but this effect was not statistically significant (p = 0.177). Conclusion FDG PET-CT scan which is a reliable imaging method to detect recurrence in CRC patients, regardless of CEA levels, can also provide valuable prognostic information, even superior to that of CEA measurement
